Found in the art-pack census of the full archive (main/srg-0494.zip :: FADE-2.TDF). The census extracted every .TDF in the 4,019-pack archive; this design's file is held byte-identical to its pack copy , and its restoration was verified against that copy.
Shipped loose in Surge's first pack, Pack Numero Uno (April 1994), with the group's ANSI art and game FAQs. Neither the pack nor the font file credits an individual artist for this font.
